I, the undersigned, hereby undertake that the above information is completely true and that I have not concealed any information. I will always dedicate myself to the Foundation and the welfare of humanity at home and abroad. I am not directly or indirectly involved in any political or government-prohibited organization. I am aware of the constitution of the Foundation and wish to become a member of this organization by paying the membership fee and subscription. I will not engage in any activity against the said Foundation or the constitution of the country. If I do so, the Foundation authorities may immediately cancel my membership, expel me or take legal action against me and the Foundation will not be responsible for my offense.
Note: I will give fixed annual grant of 1200/- and 300/- for ID card and other expenses totaling 1500/- which is eligible for donation for running the foundation and helping the needy people.
